COMMUNITY ENGAGEMENT -DD DAY SUPPORT NON CENTER BASED SERVICE

   Guiding Paths service Intellectual Disability a MR Adult Service Provider provides a variety of services to meet the needs of individuals who are diagnosed with an Intellectual Disability or Mental Illness diagnosed.




Services Provided 

 

  • Homes that have security camera with offsite video storage and secured locks with a security system that automatically calls for emergency assistance if needed.
  • Fully furnished rooms in a community
  • A diverse nutritional plan that includes fresh fruits and vegetables 
  • Cooking courses
  • Daily support to the client to enhance their independent and community living skill
  • Individualized Service Plan clearly documenting the consumer's need for specialized supervision. 
  • One on one support service to help clients in social growth and other areas

  • One on one mentoring

Eligibility Requirements and Admission


  • Consumer must be at least eighteen years of age;
  • Consumer must have a primary diagnosed of a developmental or intellectual disability (IQ between 50 and 70) and may have a secondary diagnosis of mental illness; 
  • Consumer must be tuberculosis free; 
  • Serve both Male and Female with developmental or intellectual disability.
  • Consumer's service needs can be managed by GUIDING PATHS
  • The consumer must have significant limitations in adaptive skills in two or more of the following areas-communication, self-care, home living, social skills, community use, self-direction, health and safety, functional academics, leisure and work;
  • Only individuals eligible for MR wavier funding qualify for service.

Guiding Paths is committed to providing community-based treatment that promotes self-determination, empowerment, recovery, resilience, health and the highest possible level of consumer participation in order that individuals can fully engage in community activities. Guiding Paths intends to improve the lives of individuals and families affected by Intellectual Disabilities and who may also have a psychiatric disability by ensuring access to clinically sound therapeutic interventions that improves adaptive functioning and quality of life. 



Services Provided 


  • To provide one on one community engagement 
  • To facilitate community relationships through various methods of advocacy and one-to-one consultation: linking consumers to support groups, linking consumers to vocational and educational opportunities; linking consumers to spiritual base organizations; assisting consumers with leisure and recreational activities to include outings to (sporting activities, parks, restaurants and religious services); and encourage family outings;
  • To access to and participation in everyday activities and events within the community
  • To provide an active life be connected in involved in life and contributing to the community
  • To participate in volunteering activities, encouragement , entertainment, career planning, health and wellness, within a christen based environment. 
  • To provide training and support to strengthen community-based living in a family like home setting - these services include but are not limited to: support in recognizing community safety issues (such as responding to a fire or using a telephone); assistance with identifying and undertake new hobbies; access community services; learning appropriate behavior and/or recognizing community norms; developing relationships and expanding circles of friends; 
  • To build and facilitate natural supports within the community daily
  • To Identify and address barriers that limit community and peer involvement and relationship building
  • To promote personal skill building into the Community Engagement program
  • To promoting safety and independence of the clients served
  • To provide skills training in the environments where the individual will live, work, recreate, and use the skills - these services include but are not limited to: budgeting, shopping; paying bills; assistance with household tasks; such as cooking and cleaning; emphasizing pro-social behavior;
  • To ensure that individuals understand that they are capable of learning skills for improving personal or group power– staff will assist consumers with developing assertiveness skills; self-awareness skills; improving self-esteem; decision making and judgment skills; as wells as the development of pro socialization skills.
  • To monitor health and physical condition and provide assistance as needed with transportation, personal care, and activities of daily living - these services include but are not limited to: transporting clients to appointments (medical, dental, day support, outings etc.); assist in personal care to include basic hygiene, grooming, appropriate clothing selections, shopping, appropriate etiquette, taking care of laundry, preparing meals, house cleaning etc.; 
  • To empower the individual to realize personal freedoms, individual rights, and responsibilities–to assist clients with these skills development staff provide training/support relative to: basic civil and human rights; morality; values and ethics; self-esteem building; and assertiveness training;
  • To include each person/family in the development of their individualized service plan from assessment through discharge and everything in-between;
  • To help individuals understand that they have free choice and can act without external guidance - these services include but are not limited to: training/support relative to basic civil and human rights; morality; values and ethics; healthy decision making
  • To empower individuals to exercise control over their life, including their illness – this means that staff actively involving clients in the day-to-day treatment planning process and providing training/support relative to basic civil and human rights; morality; values and ethics; self-esteem building; assertiveness training, healthy decision making
  • To help individuals gain the ability and capability to adjust to misfortune or change – an emphasis of this goal will be engaging clients in the development and clients of a crisis plan.
  • To assist in facilitating the growth of individual’s decision-making power. This will be accomplished through role playing, modeling, storytelling and the use of scenarios; additionally, ongoing processing of real life experiences will help accomplish this task.
  • To help individuals gain access to information and resources for making healthy decision – this will be accomplished through psycho-educational groups, use of the media (videos, books, pamphlets, story-telling and the processing of real life experiences;
  •  To help individuals understand that they have a right to be assertive and encourage them to think positively relative to their ability to change – this will be accomplished through psycho-educational groups, the use of the media (videos, books, pamphlets, storytelling and processing of real life experiences.
  • To assist individuals in developing positive self-images and overcome stigma – this will be accomplished through individual and small group discussions, role modeling, processing of multimedia to include storytelling, self-reflections, unconditional positive regard;
  • To facilitate moral development relative to right and wrong – story telling, modeling, individual and small group discussion and the processing or real life and fictions life experiences;
  • As needed, staff will provide training and assistance in helping clients develop adaptive healthy living skills

Guiding Paths now provides Mental Health Skill Building service for adults, ages 18 and older, with a mental health diagnosis.  Our highly trained staff is committed to facilitate opportunities that is geared to educate individuals on all available resources and services  available to them. We customize our program to service the needs of each individual   to help them achieve the highest level of stability and independence within their capabilities. Each service provided within this program is designed to improve functional skills, appropriate behavior, daily  living activities, use of community resources, as well as health nutrition and physical condition.

Eligibility Requirements and Admission


  • Have a serious mental illness;
  • Require individualized training to acquire basic living skills, which could include symptom management, adherence to medication regimen, development and appropriate use of social skills and personal support system, personal hygiene, food preparation, or money management;
  • Have a history including psychiatric hospitalization, residential crisis stabilization, Intensive Community Treatment (ICT) or Program of Assertive Community Treatment (PACT), placement in a psychiatric residential facility or Temporary Detention Order; and
  • Have a prescription for anti-psychotic, mood stabilizing, or antidepressant medications within the 12 months prior to assessment.
  • The individual shall have at least one of the following as a primary mental health diagnosis.

                                        (a) Schizophrenia or other psychotic disorder as set out in the DSM-5 

(b) Major depressive disorder;

              (c) Recurrent Bipolar-I or Bipolar II; or

                                       (d) Any other serious mental health disorder that a physician has documented specific to the identified individual within the past year and that includes all of the following: 

                         (i) is a serious mental illness or serious emotional disturbance;

            (ii) result in severe and recurrent disability; 

                               (iii) produces functional limitations in the individual's major life activities that are documented in the individual's medical record; and 

                                 (iv) requires individualized training for the individual in order to achieve or maintain independent living in the community.

Guiding Paths homes have fully cleaned and sanitized quarantine room. Designated for clients who have COVID-19 and require special care while they recuperate. Rooms are fully stocked with medical supplies and separate equipment to reduce further spread of the virus. Rooms are loaded with entertainment like computer, TV, games and other goodies to keep your loved ones in good spirits as they recuperate. 


Guiding Paths homes have fully cleaned and sanitized quarantine room. Designated for clients who have COVID-19 and require special care while they recuperate. Rooms are fully stocked with medical supplies and separate equipment to reduce further spread of the virus.
